Initial Comments:
An off-site follow-up survey conducted on May 8, 2023, found that Bridginglife, Inc., had corrected the deficiencies cited under the requirements of 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care per full implementation of the plan of corrections. The deficiencies were cited as a result of a re-certification survey completed on February 27, 2023.
